P New Patient Registration

We hope your pet will become part of our family!

If you are ready to join our family of clients and patients, we look forward to meeting you!  Please call our office number to arrange a specific appointment time. To expedite the initial client and patient registration, please fill in and submit the form below prior to your appointment.

First Visit Tips:

  1. During your first visit with us, we will get to know you and your pet. Please bring any medications that your pet is taking and any medical records that you might have from a previous provider. Alternatively, you can have your previous veterinarian send those medical records directly to us. Via email or via fax (828-658-1812).

    Please arrive fifteen minutes prior to your scheduled appointment time to allow for paperwork and charting.
  2. If you adopted your pet from a local animal shelter within the last thirty days, we will provide an initial well-animal examination and consultation at no charge. (Please note that this service is for well animals to get established with our practice. If the pet is sick at the time of the visit, normal fees will apply. Also, any additional tests or services that are decided upon during this consultation are not included in the free exam.)
  3. We encourage you to fill out the new client/new patient form on this page and submit it PRIOR to your appointment time to facilitate and expedite your visit.

    If it is more desirable or convenient for you, please print the form using the link below and bring the printed form with you to your appointment.
  4. Payment options include all major credit cards, cash, check, Scratchpay, or Carecredit.

    Payment is required at the time that services are rendered.  If you have any concerns about fees or payments, please discuss these with our staff before or during the appointment. We strive to keep care affordable and will make every effort to ensure that you are fully informed of the cost of whatever treatment plan is recommended.
